Once you are invited to apply, you will receive an email with your username and a link to the USC Supplier Portal to begin the registration process.

Both PO Suppliers and DV Suppliers are required to provide certain details about their business to assist USC in the screening process. You will be prompted during the registration process to include the information and upload the documentation appropriate for the type of business you plan to do with the university.

Preparing to apply

Both PO Suppliers and DV Suppliers will need the following information to begin the online registration process:

  • Legal business name, including any “doing business as” (DBA) if you do business under a name other than your legal one
  • Address and contact information, including at least one physical street address—not a post office box (see Addresses, Contacts and Locations for further information)
  • Federal Employer Identification or Social Security number
  • Legal business structure (company type or individual/sole proprietor)
  • Primary North American Industry Classification System (NAICS) Code (the national classification of businesses by type); keywords can be used to locate the code(s) most appropriate for your business
  • Tax information
    • W-9 or foreign tax form
    • California Form 590 (for individual nonresident service providers claiming full or partial exemption from California income-tax withholding)
  • Bank account information for direct deposit
  • Address and email contact for receipt of payments
  • Potential conflicts of interest such as close relations (spouse, domestic partner, fiancé, significant other, parent, grandparent, child, grandchild, sibling, aunt, uncle, niece, nephew, cousin, etc., including in-laws and step-relations) and financial dependents who are USC employees

PO Suppliers may also need the following information:

  • Number of employees
  • Annual revenue
  • Date business was established
  • Insurance information (type, limit, expiration date, provider) and PDF certificate (proof of current coverage naming USC as additional insured)
  •  Dun and Bradstreet number

Diverse/small business suppliers will have the opportunity to specify their classifications and upload certifications. Definitions of diverse supplier classifications and any required certifications are available on the USC Small Business Diversity Office page.

Commodity-specific requirements

Suppliers providing certain commodities may be required to upload additional documentation or complete additional assertions:

Caterers

Depending on the type of services you will provide, you may be required to provide copies of your:

  • County of Los Angeles Public Health Operating Permit
  • California State Board of Equalization Seller’s Permit and/or
  • State Department of Alcoholic Beverage Control Alcoholic Beverage License

Contractors

You will need to:

  • Upload a PDF copy of your California contractor’s license
  • Submit PDF copies of your three most recent financial statements, including income statements and balance sheets
  • Provide details on three to five construction projects within the past two years using the Construction Project Experience form (see Related Documents, located at the bottom of this page)

Medical enterprises

If you provide goods or services to any of USC’s medical enterprises (dentistry, hospitals, medicine, pharmacy), you will be required to:

  • Upload Health Insurance Portability and Accountability Act of 1996 (HIPAA) privacy compliance forms and/or
  • Submit Stark II compliance information
  • Register with REPtrax, a web-based service that credentials and monitors sales/service representatives in USC’s hospitals, if you will enter Keck Hospital, Norris Cancer Hospital or Verdugo Hills Hospital

Printers

If you are a printer, you will be asked to upload:

Security addendum

If you will access university information, you must sign a security addendum.

University marks

If you will reproduce any university marks, you must become licensed to do so.

Completing the online registration

When you have gathered all of the above information, you are ready to begin. The step-by-step guides under Related Documents (located at the bottom of this page) and the video below will help you work through the online registration.

You will be able to save your registration and return to it later if you are unable to complete it in one sitting. You also will have a chance to review your registration before submitting it. If you have neglected to complete any required section, it will be flagged to aid you in fully completing your registration.

After you complete and submit your registration, you should hear back within five (5) business days with an email indicating one of the following:

  • We need additional information to complete the screening process.
  • Your registration has been accepted.
  • Your application has been declined, including the reason for rejection.
